Transitions Community Library

Our vision is to create the largest community library where individuals, families, communities, professionals, academics and policy makers can come to find the latest insights, policies and educational resources about transitions.

This community library is a platform for free exchange of information and resources. We invite everyone to contribute to it, in particular we are keen for professionals to consider creating their own resources along with using other’s resources. You will be acknowledged as the author of that resource and we will also have awards under different categories, e.g., most effective transitions practice. Similarly, academics and policy makers are also invited to submit any (freely available) literature and other materials of interest that you own copyright to.



The library will include:

  • research articles, including those based on Jindal-Snape’s MMT Theory and XII Pillars
  • transitions educational resources (e.g., comics, games, story books), videos and podcasts
  • transitions guidance/policy material
  • best practice case studies/reflections/learning
  • ask the expert and top tips

We will also provide guidance and templates for evaluation of your own practice, especially when you have used any educational resources from the Transitions Community Library. We just ask that you provide anonymised results so that others can also understand which resources might be effective for whom and why.
