Procedure for staff or student in mental distress
If this is an emergency

Incident Report 1- contains personal information
Consent must be obtained for completion of this form.
It should then be submitted to the University Health Service by the reporter, with no other copies being retained.
On receipt of this form, University Health Service will contact the identified person via email or letter, to highlight possible sources of support, including the University Health Service itself.
No further actions will be taken unless the person concerned initiates contact, or the information supplied raises health professionals’ concerns, whereupon actions in accordance with their own professional obligations will be taken.

Incident Report 2 - does not contain personal information
This form should be used when either:
- a distressed person’s situation has been identified as being an emergency and s/he has been engaged with statutory services,
or
- their situation is not considered to constitute an emergency, but they do not provide consent to Incident Report 1 being completed.
It should then be submitted to the University Health Service by the reporter, with no other copies being retained.
No further actions will be taken unless the information supplied raises health professionals’ concerns, whereupon actions in accordance with their own professional obligations will be taken.
