Get help with health costs

Updated on 22 January 2021

If you are on a low income, you could get some help with costs through the NHS Low Income Scheme

On this page

A low income is described as having less than £16,000 in savings, investments, or property.

The scheme will provide help for cost of:

  • NHS dental treatment
  • sight tests, glasses, and contact lenses
  • NHS wigs and fabric supports

(Private dental costs are not covered).

Download application forms

If you have not had treatment yet, use the HC1 form to provide a certificate detailing what costs if any you will have to pay for.

If you have already paid for treatment use the HC5 form to claim back expenses, you will need to be able to provide a receipt for the treatment.

You can download both forms and get further information from the NHS website.
