Private Consultancy and Personal Payment forms

Staff should use the following forms when wishing to carry out private consultancies or to undertake research or service contract work involving a personal payment.

Please note: The private consultancy form should be returned to the University Secretary once signed by your Head of Department.

The Private Consultancy and Personal Payment forms are interactive PDFs and can be opened and saved using Adobe Acrobat Standard.

Alternatively, RIS's own software Get Approved can be used to open and save the forms in a similar way to Acrobat Standard.
