What is a modified short synacthen test?

This test is performed to check the amount of cortisol and a special hormone called 17 hydroxy progesterone in your body and to check how well your body can produce cortisol.

What is cortisol? What is 17 hydroxy progesterone?

Cortisol is one of the essential steroid hormones to keep you healthy. 17 hydroxy progesterone is an intermediate product in the production of cortisol which is present at very low level in a female, but can be raised in the genetic disorder called congenital adrenal hyperplasia.

What do I need to do for the test?

It will be better to have the test in the morning. You will be requested to remain in bed rest during the test. You should not have the test if you have significant asthma.

What will happen on the day of the test?

You will be asked to lie flat for the duration of the test. Your named nurse will introduce herself and explain what to expect.

A small needle/canula will be inserted into a vein in the back of your hand or arm. This is called a venflon and allow your named nurse to obtain blood samples without causing too much discomfort.

We will take a blood sample to check the baseline level of hormones. We will then give you an injection to stimulate your own body production of cortisol. A blood sample will be taken at 30 minutes and 60 minutes after the stimulation to measure the cortisol and 17 hydroxy progesterone level.

If there is a partial blockage in production of cortisol, 17 hydroxy progesterone may be high at the outset and increase to even higher levels during the test.

AFTER THE TEST

On completion your venflon will be removed and you will be offered refreshments prior to going home.

Who will get the results?

Your doctor will be informed of the results. Measurement of 17 hydroxy progesterone canl take 3 to 4 weeks before we can get the result.
