The schedule of registration fees is as follows:

These payments include access to all scientific sessions, congress materials, morning coffee and afternoon tea on all three days of the congress, one ticket to the Welcome Reception and VAT where applicable. Lunch is NOT included in the registration fee however cash catering will be available.   All registration forms MUST BE accompanied by the appropriate payment.

 **IUA Member (application pending)

The IUA encourages new members with an interest in any aspect of vascular disease.  Any person joining the IUA at the time of registration will be able to register at the IUA Member Registration Fee.  To receive an application form, please go to: www.I.U.Angiology.org/anglais/applications.html and for further information of joining the IUA, please visit: www.I.U.Angiology.org.  Please return the Registration and Accommodation Booking Form to the Congress Secretariat.  The IUA membership application form should be returned to the IUA.  Your registration form and payment will NOT be processed until your IUA membership application has been successful, there will be a delay in confirmation whilst membership is being processed.  It is not possible to register at this rate after the 2^nd August 2005 due to time constraints.

Method of Payment

To register to attend the meeting please click HERE to register on-line via our secure credit card site.  Alternatively, please fully complete the downloadable Registration Form and return to the Secretariat together with the requisite fee.  Payment must be made in UK £.  Payment can be made either by sterling cheque (payable to “Royal Society of Medicine”) and drawn on a UK bank, credit card (Visa, MasterCard or American Express) or debit card (Switch, Maestro or Visa Delta).  Credit card payments are subject to an additional charge (MasterCard/Visa/Amex: 2.95%).  Credit card payments cannot be taken over the telephone as written authorisation is required.  All payments will be debited in UK £.  If your credit card currency is not in UK £ the payment will be converted by your card issuer using their exchange rate.  Therefore the amount showing on your statement will not be exactly the amount shown here as a guide.  Places at the meeting cannot be reserved in the absence of a registration form and payment.

 Confirmation of Registration

Written confirmation and a receipt of payment will be sent to you upon receipt of your registration form. Further details and directional maps will be forwarded to you shortly before the congress.

 Cancellations of Registration

The registration fee will be refunded with a 25% deduction, provided that written notice is received before 27^th September 2005. No refunds will be made for cancellations received after this date, although name substitutions will be accepted at any time. All refund payments will be made after the congress has taken place.

Please note that all on-line payments are subject to a credit / debit card charge of 2.95%.  Unfortunately we are unable to accept on-line payment using Amex.
